Greater Hewitt Chamber Membership Application
Thank you for your interest in the Greater Hewitt Chamber of Commerce membership! Member dues are based on number of employees and presented to the Greater Hewitt Chamber Board each second Wednesday for acceptance and approval. Chamber membership remains in effect until cancelation is sent in writing. Dues are non-refundable and subject to change annual by the Board of Directors.

By submitting my application, I accept the responsibilities and expectations of a member described at Hewittchamber.com/join in the application process section. Once memberships are approved, members promise to support GHCOC’s purpose, as stated in the Mission Statement. Since 1978, our mission has been to promote an environment of goodwill and prosperity among businesses, residents and civic organizations in the greater Hewitt area. Have you or the company which you represent, if any, ever been convicted a crime in which a final conviction was entered and in which you or the company you represent were charged with committing an act or acts of fraud or moral turpitude or which resulted in death, serious bodily injury or property loss? If yes, please give details including the date, county and state of the criminal action, the cause or case number of the case, whether the crime was a felony or a misdemeanor and a general description of the case. This information should be supplied regardless of whether probation was given or a pardon or clemency was granted.
Have you or the company which you represent, if any, been a party to a lawsuit in which a final judgment was entered either against you or the company you represent and in which there were findings by a judge or jury of fraudulent or deceptive business practices? If yes, please give details including the date, county and state where the lawsuit was filed and judgment entered, the cause or case number of the case and a general description of the case. This information should be disclosed regardless of whether or not the case was settled and judgment dismissed.
